The Planned Care for Better Health Coordinator will work autonomously within Manning Hospital, as well as with health services, primary care and community service providers across the LMNC Sector. This dynamic new role will facilitate the provision of integrated care for vulnerable patients at risk of hospitalisation or emergency department presentation, championing collaborative care approaches with patients, families, carers and multidisciplinary teams to achieve identified goals and outcomes. We are seeking a skilled Physiotherapist Level 3, Occupational Therapist Level 3, Dietitian Level 3, Social Worker Level 3, Speech Pathologist Level 3 or Registered Nurse to step in this pivotal role on a part time basis!

 The opportunity:
As the Planned Care for Better Health Coordinator, you will help facilitate a coordinated team approach that seeks to optimise client wellness at home while reducing unnecessary hospital admissions and emergency department presentations. The role will include reviewing identified patients current health service utilisation, providing care coordination, care navigation and health coaching. You will be supported in this role by the LMNC Sector Director, Allied Health and Integrated Care; as well as by the Hunter New England Integrated Care Program Manager. You will be provided with orientation and mentoring within the role, as part of an individualised professional development plan. 

About you:
You will be a highly experienced, motivated Nursing or Allied Heath Clinician; with high level communication and interpersonal skills, and experience in facilitating patient centred care and advocacy for people who are living with a chronic disease or complex need. You will be committed to shared decision making with patients and their families / carers, thrive on complex case planning; and enjoy working in partnership with service providers across the health and community care continuum.
